ABSTRACT

Objective:To observe any curative effect of combining hydraulic dilatation with radial extracorporeal shock waves on scapulohumeral periarthritis.Methods:A total of 132 patients with scapulohumeral periarthritis were randomly divided into a hydraulic dilatation group, a shock wave group and a combination group. The three treatments were applied for 3 weeks, before and after which shoulder range of motion was evaluated. Shoulder function was quantified using the Japanese Orthopaedic Association′s (JOA′s) shoulder function scoring, pain was reported using a visual analogue scale (VAS), and ability in the activities of daily living (ADL) was also quantified. Serum prostaglandin E2 (PGE2), β-endorphin (β-EP), interleukin 6 (IL-6) and tumor necrosis factor α (TNF-α) were assayed.Results:The treatment response rate of the combination group (95%) was significantly higher than those of the hydraulic dilatation group (70%) and the shock wave group (82%). After the treatment, the average shoulder range of motion, JOA and ADL scores, and β-EP level in all 3 groups had improved significantly, while the average VAS score, and the PGE2, IL-6 and TNF-α levels had decreased significantly, but in each case the improvement in the combination group′s average was significantly greater than those of the other 2 groups. During the treatment there were no such adverse events as intra-articular infection, skin redness, numbness or palpitations.Conclusions:Combining hydraulic dilatation with radial extracorporeal shock waves significantly improves the treatment of scapulohumeral periarthritis. It can effectively promote the recovery of shoulder function and improve quality of life. Its greater effectiveness may be related to improving levels of PGE2, β-EP, IL-6 and TNF-α, and relieving pain and inflammation.

ABSTRACT

OBJECTIVE: To observe the clinical effect of "metal hook fishing" (MHF)-like needling technique combined with Tuina manipulation in the treatment of scapulohumeral periarthritis. METHODS: A total of 72 patients with scapulohumeral periarthritis were randomly divided into MHF-like needling (MHF) and conventional needling groups (n=36 in each group). For patients of the GHF group, the doctor pressed the acupoint with the left index finger, held the disposable acupuncture needles to insert into the Jianqian, Jianyu (LI15), Jianliao (SJ5), Jianzheng (SJ9) and Ashi-point respectively with the right hand, waited for qi-arriving, twirled the needles forward about 3 to 6 times. When a tightened feeling was achieved beneath the needle tip, the doctor held the needle body to slightly shake it 3 to 6 times which mimicked a fish to swallow a hook, kept the situation for a while till relaxation, followed by retaining the needle for 30 min and then withdrew it slowly. Afterwards, the doctor performed Tuina manipulation at the upper limbs for 20 to 30 min. For patients of the conventional needling group, the same acupoints were needled and stimulated with the uniform reducing and reinforcing method for 1 min, followed by retaining the needles for 30 min. Then, massage was performed for 20 to 30 min in the same way as those in the MHF group. The treatment was conducted once daily, 6 times a week for 2 weeks. After the treatment, the visual analogue scale (VAS) was used to assess the pain degree, the range of motion (ROM) scale of the shoulder joint adopted to assess the function, and the activity of daily living (ADL) scale used to assess the patients' daily living ability. The therapeutic effects of the two groups were evaluated after the treatment. RESULTS: After the treatment, the VAS scores of the two groups were notably decreased (P<0.05), being significantly lower in the GHF group than in the conventional needling group (P<0.05). The ROM score and ADL score of the two groups were obviously increased (P<0.05), being significantly higher in the MHF group than in the conventional needling group (P<0.05). Of the 36 cases in the conventional needling and MHF groups, 7 and 17 were cured, 11 and 12 experienced remarkable improvement, 13 and 4 were improved, and 5 and 3 were ineffective, with the total effective rate being 86.1% (31/36) and 91.7% (33/36), respectively. The total effective rate of MHF group was evidently higher than that of the conventional needling group (P<0.05). CONCLUSION: Both MHF and conventional needling is effective in relieving scapulohumeral periarthritis, and the effect of MHF is ob-viously superior to that of conventional needling in relieving pain state and improving function of the affected shoulder joint.

OBJECTIVE@#To explore the effective therapy for treating chronic scapulohumeral periarthritis of cold-damp stagnation.@*METHODS@#A total of 90 cases of patients with chronic scapulohumeral periarthritis of cold-damp stagnation were randomly divided into an acupuncture and moxibustion group, a herbal cake separated moxibustion group and a routine rehabilitation group, 30 cases in each group. The routine rehabilitation group was treated with diclofenac sodium sustained-release tablets (0.1 g each time, taken after breakfast) and rehabilitation exercise, once a day. On the basis of the treatment in the routine rehabilitation group, the herbal cake separated moxibustion group was treated with herbal cake separated moxibustion at the affected side of Jianyu (LI 15), Jianliao (TE 14) and Jianzhen (SI 9), once a day. On the basis of the treatment in the herbal cake separated moxibustion group, the acupuncture and moxibustion group was additionally given umbrella shaped acupuncture with round sharp needle at the affected side of Jianyu (LI 15), Jianliao (TE 14), Jianzhen (SI 9), Naohui (TE 13), Jianqian (Extra), Jugu (LI 16), etc. once every other day. Each group was treated for 10 d. Before and after treatment the pain visual analogue scale (VAS) score and activities of daily living (ADL) score, and degree of changes in shoulder joint activity were compared in each group, and the clinical effect was evaluated.@*RESULTS@#After treatment, the pain VAS scores of three groups were decreased (@*CONCLUSION@#On the basis of routine rehabilitation training, herbal cake separated moxibustion combined with umbrella shaped acupuncture with round sharp needle treating chronic scapulohumeral periarthritis of cold-damp stagnation can significantly reduce shoulder joint pain and improve shoulder joint function.

ABSTRACT

OBJECTIVE: To observe the clinical therapeutic effect of Fuyang-pot warming combined with electroacupuncture (EA) in the treatment of scapulohumeral periarthritis (SPA). METHODS: A total of 90 cases of SPA patients were randomized into EA, Fuyang-pot warming and EA plus Fuyang-pot warming (combination) groups (n=30 per group). Fuyang-pot warming including pressing, mild moxibustion, scraping-pushing, cupping, tapping, etc. was applied to Fengchi (GB20), Dazhui (GV14), Jianjing (GB21), Jianyu (LI15), Zhongfu (LU1), Ashi-point, etc., and EA (2 Hz /100 Hz,1-1.5 mA) was appled to GB20, GV14, GB21, LI15, Binao (LI14), Tiaokou (ST38), Chengshan (BL57), Ashi-point, etc. The treatment was performed for 30 min every time, once every other day for 2 weeks. The visual analogue scale (VAS, 0-10 points) was used to assess the pain severity. The Constant-Murley shoulder assessment scale (100 points in total, including 15 points in pain severity and 20 points in daily living activities, 40 points in joint motion range, and 25 points in myodynamia) was used to assess the functional state of the shoulder. The rating scale of the American Shoulder and Elbow Surgeons (ASES, 4 grades) was used to evaluate the ability of daily living activities. RESULTS: Following the treatment, intra-group comparison showed that the VAS score was significantly reduced in the three groups in comparison with their own pre-treatment (P0.05). CONCLUSION: Fuyang-pot warming combined with EA is effective in relieving pain, and enhancing the daily life quality in scapulohumeral periarthritis patients.

ABSTRACT

Objective To investigate the MR features of different clinical staging of scapulohumeral periarthritis and provide relevant support data for the clinical staging of scapulohumeral periarthritis, so as to guide clinical treatment. Methods 30 patients with scapulohumeral periar-thritis received in the hospital from June 2015 to February 2017 were selected to form the observation group and 8 same-sex and same-aged volunteers without such disease were selected to form the control group. MR imaging was used to observe and measure the structure of shoulder joint of the people in the two groups and statistical analysis was performed to analyze the changes in the structure of the shoulder around dif-ferent clinical stages. Results The thickness of joint capsule and coracohumeral ligament ( CHL) , the ratio of subcoracoid fat triangle re-placed were significantly greater in patients with scapulohumeral periarthritis than those in the control group, and the difference was statisti-cally significant (P<0. 05). Comparing the thickness of joint capsule in the third stage of the scapulohumeral periarthritis group compare with those in the first and second stages, the difference was statistically significant (P<0. 05). There was no statistically significant differ-ence (P>0. 05) in the thickness of the coracohumeral ligament and the ratio of subcoracoid fat triangle replaced in the scapulohumeral peri-arthritis group between the 1st, 2nd and 3rd stages. Conclusion Patients with different stages have different structures around the shoulder joints. The thickness of joint capsule can be used as an important reference for diagnosing scapulohumeral periarthritis and can guide the clin-ical staging. The thickness of coracohumeral ligament and the ratio of subcoracoid fat triangle replaced can be used as a basis for diagnosing scapulohumeral periarthritis, but it cannot be used as a guideline for clinical staging.

ABSTRACT

Objective To investigate the efficacy of acupuncture at mind-tranquilizing points plus conventional shoulder points in treating scapulohumeral periarthritis (periarthritis of shoulder) at the early stage of adhesion. Method Sixty patients were randomly allocated to two groups. The treatment group received acupuncture at mind-tranquilizing points plus conventional shoulder points and the control group, at conventional points. In both groups, treatment was given three times a week, for two weeks. The Hamilton Anxiety Scale (HAMA) score, the Hamilton Depression Scale (HAMD) score, the Japanese Orthopaedic Association (JOA) score and the Visual Analogue Scale (VAS) score were recorded before and after treatment. Result The VAS score,the HAMA score, the HAMD score and the JOA score improved in both groups after treatment compared with before (P0.05). Conclusion Mind-tranquilizing acupuncture plus conventional acupuncture is more effective than conventional acupuncture in treating scapulohumeral periarthritis at the early stage of adhesion.

ABSTRACT

Objective To explore the respective advantages of different direction Hegu needlings by using them to treat the pain stage of scapulohumeral periarthritis (periarthritis of shoulder) and investigating their clinical therapeutic effects. Method A randomized controlled trial was carried out. Sixty patients with scapulohumeral periarthritis in the pain stage were allocated into group Ⅰ (30 cases) and group Ⅱ (30 cases). Group Ⅰ received acupuncture at points Jianyu(LI15), Jianliao(TE14) and Jianzhen(SI9) in the direction parallel to the meridian course and group Ⅱ , in the direction perpendicular to the meridian course. Treatment was given 30 min once for a total of 12 times. The simplified Mcgill Pain Questionnaire score and the Japanese Orthopaedic Association (JOA) score were recorded before and after treatment. Result There were no statistically significant differences in the pain, joint activity and joint function between the two groups before, during and after treatment. Conclusion Two different direction Hegu needlings both have a marked therapeutic effect on the pain stage of scapulohumeral periarthritis. The two have no significant difference.

ABSTRACT

Objective To observe the clinical efficacy of fire-needle therapy in treating scapulohumeral periarthritis, and to observe the changes of pain score and the motor function of shoulder joint. Method Totally 180 patients were randomized into a fire-needle therapy group of 90 cases and a filiform needle group of 90 cases by randomized single-blinded method. Result There were significant differences between the two groups in comparing the recovery rate, motor function of shoulder joint, and the relapse rate 30 d after the whole intervention (P0.05). Conclusion Compared to filiform needle therapy, fire-needle therapy can produce a better recovery rate and motor function of shoulder joint in treating scapulohumeral periarthritis.

ABSTRACT

Objective To investigate the clinical efficacy of acupoint application plus massotherapy for scapulohumeral periarthritis (periarthritis of shoulder). Method One hundred and two patients with scapulohumeral periarthritis were randomly allocated to treatment and control groups, 51 cases each. The treatment group received acupoint application plus massotherapy and the control group, massotherapy. Both groups were treated for three courses. The VAS score and the Neer shoulder function score were recorded in the two groups before and after treatment. Result The VAS score and the Neer shoulder function score improved in the two groups after treatment compared with before (P<0.05) and improved more in the treatment group than in the control group (P<0.05). Conclusion Clinically acupoint application plus massotherapy has a satisfactory therapeutic effect on scapulohumeral periarthritis with a certain effect on pains in the affected shoulder.

ABSTRACT

Objective To observe the efficacy of Zheng’s “gold hook fishing acupuncture” in the treatment of scapulohumeral periarthritis. Methods Sixty patients were randomly divided into gold hook fishing group and reinforcing-reducing group, with 30 cases in each group. Gold hook fishing group chose the main points from the Tianzong (SI11), Bingfeng (SI12), Jiajixue (double EX-B2), Binao (LI14), Jianyu (LI15), Jianliao (SJ14), Jianzhen (SI9), Ashi, Naoshu (SI10), Quchi (LI11), Waiguan (SJ5), Hegu (LI4), Houxi (SI3), Tiaokou (ST38) and Chengshan (BL57). Needles were pierced at tendon junction points and Ashi points, and golden hook fishing acupuncture was operated. Other points were given reinforcing-reducing method. The reinforcing-reducing group was treated with reinforcing-reducing method at points the same as gold hook fishing group. Patients were treated once a day for 2 courses, 5 days as a treatment course. The pain visual analogue scale (VAS), shoulder range of motion (ROM), and ability of daily life (ADL) were observed before and after treatment to evaluate the clinical efficacy. Results The total effective rate of gold hook fishing group was 90.0% (27/30), better than the reinforcing-reducing group 83.3% (25/30, P<0.05); VAS pain score, ROM score and ADL score before and after treatment in the two groups were improved to some extent (P<0.05), and scores in the golden hook fishing group were improved more significantly compared with reinforcing-reducing group (P<0.05). Conclusion Zheng’s gold hook fishing acupuncture can effectively improve the clinical symptoms and signs of patients with scapulohumeral periarthritis and their life quality.

ABSTRACT

Objective To observe the curative effect of warm needling therapy combined with kidney-supplementing and blood-activating Chinese herbal medicine on patients with scapulohumeral periarthritis and its effects on immunoglobulin IgA, IgG and IgM;To explore the mechanism. Methods Ninety patients with scapulohumeral periarthritis were randomly divided into needle with medicine group, needle group and medicine group, 30 cases in each group. Patients in the needle with medicine group were treated with needle warming therapy combined with kidney-supplementing and blood-activating Chinese herbal medicine, patients in the needle group were treated with needle warming therapy, patients in the medicine group were treated with kidney-supplementing and blood-activating Chinese herbal medicine, 30 days for medicine and every other day for acupuncture. Five time acupuncture was one course of treatment, and patients were treated for 3 courses. Shoulder pain index was evaluated, and the levels of IgA, IgG and IgM was determined before and after 3 courses treatment. The total curative effect of the three groups were compared. Results The total effective rates for needle and medicine group, needle group and medicine group were 93.33% (28/30), 73.33% (22/30), 66.67% (20/30), respectively, with statistical significance between needle and medicine group and the other two groups (P0.05). There was statistical significance between the shoulder indexes before and after treatment of the three groups (P<0.05). Before treatment, the levels of IgA, IgG and IgM of all three groups increased significantly compared with the normal reference values (P<0.05). After treatment, the levels of IgA, IgG and IgM were reduced significantly in three groups after treatment (P<0.05), the levels of IgA, IgG and IgM in the needle and medicine group were significantly lower than those in the other two groups (P<0.05). Conclusion The curative effect of needle warming therapy combined with kidney-supplementing and blood-activating Chinese herbal medicine was better than alone needle warming therapy or kidney-supplementing and blood-activating Chinese herbal medicine. This combined therapy could significantly improve the symptoms of patients, reduce the levels of IgA, IgG and IgM, regulate the body immune function, which is probably one of the mechanisms in the treatment of scapulohumeral periarthritis.

ABSTRACT

Objective To investigate the efficacy of wrist three-needle acupuncture in treating pre-adhesive stage scapulohumeral periarthritis. Method Seventy-eight patients with pre-adhesive stage scapulohumeral periarthritis were randomly allocated to two groups. The treatment group of 39 patients received wrist three-needle acupuncture and the control group of 39 patients, conventional acupuncture. Result After one course of treatment, the cure rate and the total efficacy rate were significantly higher in the treatment group than in the control group and there was a statistically significant difference (P<0.05). There was a statistically significant pre-/post-treatment difference in the VAS score in the treatment group (P<0.05). There was a statistically significant difference in the pre-/post-treatment difference value between the treatment and control groups (P<0.05). Post-treatment pain relief rate was significantly higher in the treatment group than in the control group (P<0.05). Conclusion Wrist three-needle acupuncture is even more effective than conventional acupuncture in treating pre-adhesive stage scapulohumeral periarthritis.

Objective] Summarize clinical experience about treatment of scapulohumeral periarthritis with neck rotation and shoulder movement proposed by the well-known doctor Wang Youren from Beijing Massage Hospital. [Method] Observe the clinical treatment of scapulohumeral periarthrit, organize the related medical records and draw on the treatment thoughts and operational method.[Result] Mr. Wang thinks that there is close connection between the scapulohumeral periarthriti and neck in the perspective of pathogenesis and anatomy and physiology. It will have better treatment effect if giving the fine-tuning to the neck while treating the scapulohumeral periarthritis. [Conclusion] Mr. Wang Youren has unique clinical treatment experience in the area of scapulohumeral periarthritis, and this treatment has significant effect. Thus it is worth of spreading.

ABSTRACT

[Objective] To observe the clinical effect of revised Duhuo Jisheng Decoction and combining therapy on scapulohumeral periarthritis. [Method] Select 150 cases for research data, among which, 50 cases were treated with Duhuo Jisheng Decoction and combining therapy as treatment group, other 100 cases with only combining therapy as control group, 15d as a course;compare their effects after 2 courses. [Result] After treatment, the comparison of pain VAS scores between the 2 groups, the difference was of statistical meaning( P<0.05); The cure rate and effective rate were respectively 78.00% and 92.00%,and for other group, they were respectively 60.00%and 85.00%;the treatment group was better than other group, the difference was of statistical meaning.[Conclusion] Revised Duhuo Jisheng Decoction and combining therapy treating scapulohumeral periarthritis pays attention to internal arghralgia treatment, and the superficial as wel , with definite cure effect, worthy emphasizing and promotion for basic hospitals.

ABSTRACT

Objective To study the load distribution of muscles for those having long period stance training during traditional chinese Gongfu stance, and evaluate the effectiveness of such stance. Method Seven traditional Chinese Gongfu stance trainers and seven normal adults for the control were enrolled in the experiment. The sEMG (surface electromyography) signals of five muscles (brachioradialis, triceps brachii, deltoid, latissimus dorsi, triceps surae) were tested by Flexcomp Infiniti 10 channel sEMG collector. The value of IEMG (integral electromyography) were processed. Results With the increase of load, the ratio between deltoid IEMG and latissimus dorsi IEMG for the stance trained group stabilized in the range from (0.372±0.258) to (0.257±0.146), while that of the control was in the range from (1.313±0.698) to (0.800±0.405). Conclusions When the traditional Chinese Gongfu stance is processed with or without load, the trained group distributed major load on the dorsal muscle, while the control on the shoulder.

@#Objective To investigate the efficacy and analgesia of moxibustion with warming needles combined with joint mobilizationon scapulohumeral periarthritis. Methods 126 subjects were randomly divided into 2 groups, the observation group received moxibustionwith warming needles combined with joint mobilization; the control group received moxibustion with warming needles only. Short-form ofMcGill pain questionnaire and effect were assessed. Results The effective rate was 98.4% and 95.2% for the 2 groups respectively(P>0.05);but there was significant differences between the 2 groups in the visual analogue scale (VAS), present pain intensity (PPI) and pain rating index(PRI) (P<0.05). Conclusion The moxibustion with warming needles combined with joint mobilization is a good effective therapy forscapulohumeral periarthritis.

@#Objective To observe the effect of propofol combined with sufentanil in manipulation release on scapulohumeral periarthritis.Methods 62 patients with scapulohumeral periarthritis undergoing manipulation release were randomly assigned to propofol group(Group P) and propofol combined with sufentanil group (Group PS) with 31 cases in each group. Group P received 2 mg/kg dose of intravenouspropofol pre-operatively and Group PS received 2 mg/kg dose of intravenous propofol combined with sufentanil of which the loadingdose was 0.1 μg/kg and the basal rate was 0.1μg/kg/h pre-operatively. Repeated propofol could be used in both groups at the dose of 0.5 mg/kg. Blood pressure (BP), heart rate (HR), pulse oxygen saturation (SpO2), pain on injection of propofol, level of anesthesia effect in operation,level of analgesia after operation, recovery time, and incidence of post-operative nausea and vomiting (PONV) were recorded. ResultsThe level of anesthesia effect in operation and the level of analgesia after operation were significantly higher in group PS (P<0.01). No painon injection of propofol was found in group PS. There was no significant difference in BP, HR, SpO2, recovery time, and incidence of PONVbetween two groups (P<0.05). Conclusion Propofol combined with sufentanil can effectively improve the anesthesia effect in operation andthe analgesia after operation.

ABSTRACT

Objective To compare the immediate effects of extracorporeal shock wave therapy with that of joint mobilization plus short wave therapy in treating scapulohumeral periarthritis, and to elucidate the mechanism of extracorporeal shock wave therapy and how it should be used. Methods Forty-seven patients diagnosed with scapulohumeral periarthritis were selected and randomized into two groups, a treatment group (n = 26) treated with extracorporeal shock wave therapy, and a control group ( n = 21 ) treated with joint mobilization plus short wave therapy. The short form of the McGill pain questionnaire (SF-MPQ) and range of motion (ROM) measurements were used to evaluate shoulder function. Results Compared with baseline, both groups improved significantly af-ter treatment, but the immediate effect in the treatment group was superior to that in the control group. Concln-sions As a safe, time-saving and effective therapy, extracorpnreal shock waves could be used to treat scapulo-humeral periarthritis.

ABSTRACT

Objective To observe the clinical therapeutic effects of BaDuanJin exercise on patients with scapulohumeral periarthritis.Methods 72 patients were randomly divided into therapeutic group and control group,with 36 cases in each group.The patients in control group were treated with electroacupuncture and those in therapeutic group were treated with electroacupuncture and BaDuanJin exercise for 4 weeks.Pain degree(VAS)and shoulder ROM(Melle Scores)were evaluated before therapy,2 weeks and 4 weeks after therapy.The patients were followed up for 3 months.Results The VAS scores and Melle scores of both groups after therapy(both at two weeks and four weeks)showed significant difference than those before thempy.The scores after 4 weeks therapy were better than those of 2 weeks therapy.The effects of therapeutic group after 3 months showed better result than controls,but without statistical difference(P>0.05).Conclusion BaDuanJin,as a mild exercise had favorable therapeutic effccts on patients with scapulohumeral periarthritis.

ABSTRACT

[Objective]To observe the intervention of Electroacupuncture at Point Jianyu(LI15) on Rabbit Model of Scapulohumeral Periarthritis for receiving possible mechanism.[Methods ]Thirty healthy rabbits(male or female) were randomly divided into three groups:scapulohumeral periarthritis model group,EA Jianyu group and Ibuprofen control group.Each group respectively were detected the concentrations of serum PGE2 before model and treatment and after treatment,and then killed them to achieve tendons of shoulder which were detected the concentrations of Hydroxyproline.And achieve tissues around shoulder which were studied by HE to investigate the changes of the histology.[Results]The concentrations of serum PGE2 had no notable difference between EA Jianyu and model group,Ibuprofen control group and model group after treatment.But it was lowered after treatment than before treatment by EA Jianyu.There were no significantly difference among the three groups about concentrations of Hydroxyproline in tendons,but Ibuprofen control group was higher than EA Jianyu group.Tissues of the shoulder were observed that EA Jianyu restrained reaction of inflammation and degenerescence of tissues,and promoted it better.[Conclusions] EA Jianyu has anti-inflammatory effect and cuts back on the degree of degenerescence of tissues on scapulohumeral periarthritis model in rabbit.
